Established in 2007, Healthy Community Health Centers (HCHC) is a Federally Qualified Health Center with the distinction of being a Patient Centered Medical Home. This model focuses on the patient as a whole person and utilizes a relationship-based approach to health care. Our providers partner with their patients to understand their unique needs, culture, values and preferences. In 2020 HCHC achieved the PCMH Distinction in Behavioral Health Integration by excelling beyond the original PCMH designation through the diligent work of our Behavioral Health providers.

We are seeking to add a talented Nurse Practitioner to our team who is passionate about patient-centered medical care. We offer flexible schedules, time and funding for CME, competitive salaries and a comprehensive benefits package.
Qualified Candidates:
- Have or are willing to obtain an unrestricted license to practice medicine in the Commonwealth of Virginia
- Experience with adult, child and adolescent populations
- DEA license to prescribe medicine
- Board certification in chosen field of practice
- Current CPR (BLS) certificate is required
- Preference will be given to candidates who are bi-lingual in Spanish, Arabic or Kurdish.
- Previous experience working in a Community Health Center environment is preferred but not required
- Cannot be sanctioned under Medicaid or Medicare

HCHC has locations in the Harrisonburg-Rockingham County area. This region is part of the beautiful Shenandoah Valley. Home to the Shenandoah National Park, Blueridge Parkway and Skyline Drive. Harrisonburg, Rockingham County and surrounding areas offer access to many outdoor activities including hiking, camping, fishing, and cycling. The area is home to James Madison University, Eastern Mennonite University and Bridgewater College.
